Requesting for Assistance with UP Mail Recovery

This document aims to guide users on how to request for assistance with UP Mail recovery. This is intended for active students and employees who have been locked out of their accounts, and for students and employees returning to the University of the Philippines – Los Baños, whose accounts have been deactivated.


How to Request for Assistance with UP Mail Recovery


Users must prepare the following documents when requesting for assistance with account recovery:

Student

a. For active/currently enrolled: scanned/soft copy or picture of ID or Form 5

b. For returning from LOA: scanned/soft copy or picture of approved LOA application form

c. For returning from AWOL: scanned/soft copy or picture of approved reinstatement

EmployeeScanned/soft copy of contract, appointment letter or endorsement from unit



User can request for assistance via this form: https://bit.ly/upmailrecform

 

Users are required to provide, as much as possible, their recovery email address or phone number, for our verification purposes. If the client is unable to give either of these details, they must make a personal appearance at the Information Technology Center office. If they cannot make a personal appearance, they can send an email to itc.uplb@up.edu.ph.

 

Users cannot request for the recovery of an account not belonging to them. If they are doing this on behalf of another user, they may email ITC for further assistance.
